How do I ask for a class to be reopened?

I called Acellus and they said to fill out a form. I am having trouble locating this form and need help please.

Requests of this nature would need to be submitted to our Management Department for review by the parent. Parents can submit their request to our Parent Support Team via the website Contact Us form: Contact Parent Support.
Requests are processed within one business week upon submission.
